Digital Passenger Locator Form – dPLF
Instructions for completing the form before arriving in Bibione
Starting 24 May 2021, all passengers entering Italy are required to complete a specific digital locator form.
The digital Passenger Locator Form (dPLF) collects information about your travel itinerary, telephone number and address of residence in Italy to enable health authorities to contact you in a timely manner, if necessary.
To complete the digital Passenger Locator Form (dPLF), simply follow these instructions:
- Go to https://app.euplf.eu/#/
- Follow the wizard to access the dPLF
- Choose “Italy” as the destination country
- Register on the site by creating a personal account with user name and password (required only for your first access)
- Confirm your account by clicking on the link in the email sent to the address you provided (required only for your first access)
- Complete and send the dPLF via the wizard
Once you have sent the form, you will receive the dPLF in pdf and QR code format at the e-mail address that you provided during registration. You will be required to present these on your smartphone (in digital format).
Each adult passenger must complete a form. Minor children can be registered on the form of their accompanying adult. For unaccompanied minors, the dPLF must be completed by the guardian before departure.
The completed digital form replaces the declaration referred to in Article 50, paragraph 1, of the Prime Minister’s Decree of 2 March 2021. The paper form can be completed in exceptional cases, i.e., only in cases of technological issues. Please note that the self-certification given to the border police must always be presented in paper version.
